Medical record requests will not be available from 7/2/19 to 7/20/19

The record request service will not be available from 7/2/19 through 7/20/19. If you need records forwarded please send your written and signed request by 7/1/2019. This can be mailed or faxed to 1-410-224-0855. Please supply all information including name, date of birth and information regarding the physician office where the records are to be faxed to and a cell phone number so that we can text you when your records have been sent.
